Consent Form
When you come in for a treatment, the first thing we ask you to do is to fill out our consent form. In this day and age where lawsuits are flying from all directions...we've gotta have a consent form.
We have done some thinking though, and we would like to do you a favor. We know that many of our clients are very busy. When you arrive at Beautiful You you'd like to get treatment and get out! Nobody wants to stand at the front desk and fill out paperwork.
So, or your convenience we have uploaded our consent form for you to print and fill out before you come see us!

Cancellation Policy
All members or non-members wanting to book an appointment will need a $25 hold on the reservation. This $25 will be applied to the total service or a refunded amount will be received if the service comes to less than the $25 amount.
All appointments must give 24 hour cancellation notice or forfeit the $25 amount.
Gratuity not included. Tips are appreciated.
